OVERVIEW OF OUR CARE PROCESS

Following initial referral, the Registered Manager will visit you in your home to discuss your needs. 
During this meeting, we will:
  • Provide information about 24hr Swift Care Service Ltd and the services we offer;
  • Agree a plan of care with you;
  • Explain the costs of your care package;
  • Perform a health and safety risk assessment;

Following the visit, we will provide you with a contract, which refers to the care plan, the costs and our terms and conditions.

After we have reached an agreement to provide care for you, you will be allocated a carer from our team who will be supporting you based on your care plan.
 
On subsequent visits your carer will monitor the effectiveness of your care plan and provide information to the Registered manager, so that your care plan can be updated and im­proved.
 
After a few visits, the Registered manager will visit you to find out your thoughts on our ser­vices and your carers, and to discuss ways of improving our service to you.
 
On a regular basis, the registered manager will visit you to do a full review of your care plan, to ensure it is appropriate to your needs.

Live-In Care

There are many reasons why people choose a live-in carer or support worker. Here are the main ones: Avoiding change – daily life can stay much the same with that extra support. Flexible care built around you – your carer helps as much or little as you like. Keeping the same routines, hobbies and interests.

Personal Care

Personal care is a broad term used to refer to supporting with personal hygiene and toileting, along with dressing and maintaining your personal appearance. It can cover, but is not limited to:

Hospital to Home

If you’ve had a stay in hospital and are ready to be discharged, getting extra support might help to boost your recovery, re-build your confidence, and so give you a better chance of living safely at home again. Services called intermediate care or re-enablement services offer this type of support.

Assessing your needs

These services can help you avoid going into hospital, by getting the right help for you, quickly, so that you can be cared for at home.

Social Companionship

Walking the dog. Going to collect pension or visiting the bank. Shopping for food, clothes or other items. Going to leisure activities. Enjoying a chat over a cup of tea. Company while doing crafts or hobbies.

Night Sitting / Sit in service

This is to provide a break for you as a main carer to your loved from your caring role. Sit service usually begin from 2 Hours. This can help when you need to go to the bank, shops and appointments.

Domestic Services

Meal preparation, shopping. This could also include: dusting, using spray cleaners, cleaning hard floors with mops or electric floor cleaners etc.

Disabilities & Health

Palliative care, Dementia, Learning disability, mental health. Personalised home care for people with Dementia. This includes Alzheimer's disease, Vascular dementia, Mixed Dementia, Dementia with Lewy bodies.

Learning disability and Mental health

If you care for a person with a learning disability or mental health illness, whether it’s your son or daughter, brother or sister, parent, partner or other relative or friend, then we may be able to help you.

Palliative care / End of Life Care

Our palliative care service supports adults who have been diagnosed with a life-threatening illness who wish to remain living in the comfort of their own homes.

Palliative care in one’s own home

Athough palliative care is most often associated with hospices, hospitals and nursing homes, there is no reason – given the right support – why someone cannot choose to be cared for in their own home.

Palliative care at home

Whilst coping with the end of a life is never easy, sensitive and skilled palliative care is all about helping everyone involved come to terms with the situation.

Continuing Health Care services

Support with your medication can be given in many ways to ensure that you are taking the right medication at the right time.
